First post here so let me start by saying hello!
Obvious disclaimer, I'm providing this info for scientific purposes only, any actions you take with your truck are your responsibility and yours alone.
I made my own mounting system for the CCM and got it used so this may not be a 100% accurate list, this is based solely on parts diagrams I could find. I will update if I find additional information. Hopefully someone can jump in here with better info. This post is mostly to show that it is definitively possible and to list the necessary FORScan changes. Assumes you already have lane keep assist and pre-collision assist.

I couldn't find the proper bumper harness, thus adding to the existing harness, this way cost me about $12 so it may be the way to go anyway.
CCM connector WPT-1198 buy a kit with pins
bumper harness pins 33012-2002 (or just buy mx150 or WPT-1599 connector kit with pins)
will need to purchase pin crimper if you don't have one.
CCM connector -- bumper harness connector
use 20awg automotive wire
Pin 1 -- Pin 4
Pin 2 -- Pin 5
Pin 3 -- Pin 6
Pin 4 -- Pin 15
---------Forscan Changes-----------
I am not a ForScan guru, these are only from personal observation and comparing different models ab data with and without ACC. Please let me know if anyone notices something glaringly incorrect. This works on my truck perfectly with no errors or dtcs (2021 F250 XLT Premium Tremor, 6.7 Powerstroke).
