Protech makes a pretty clean product. Biggest problem that I have with the racks is that most vendors won't build them where they will block the 3rd brake light due to legal issues. Typically, that leaves them too high for me. Some trucks look really good with them though.

Headache rack

I really like protech. the lifted ford has one that's for a 2007 dodge the stock height ford has a rack for a 90s ford. The nice thing with those models is they fit flush.

This is off their website but you get the picture. I like the aluminum but I don't like the maintenance that comes with keeping them polished and looking nice and shiny like they look in the pics. We get harsh winters up here and it plays havoc on the polished aluminum.

headache rack

For the pic in 1st post: not custom, but the manufacturer's name has left my mind. I know I've seen them around on a few trucks.

I made my headache rack and bedrails a couple years ago during winter break.

Well if it comes mind please let me know! I'm loving that set up on that truck. I also like the duel tool boxes but I pack up my dirt bike almost every weekend and don't think I'd like the limited space

If you don't like the maintance of aluminum, powdercoat is always a option.
