The chances of getting four bad injectors is slim, but getting all four in the same bank is even slimmer. Do you have a known good PCM and IDM you could swap in? I know you said the IDM is new/remand. Check to make sure there's no chip plugged into the PCM. Sounds like you have a real head scatcher here.
