First thing i would do is with the key off i would make sure you can move the actuator freely, If you can't then that means either the actuator is hung up, or the vanes in the turbo are full of carbon or the ring in the housing has frozen up. If it moves freely then the actuator and turbo are moving freely but the motor or something isnt working right in the actuator, I have an actuator i could probably send you to try out and make sure it fixes it, if it does you can just send it back.
