Exhaust pressure sensor burnt off

Soo, a code came up on my mini maxx pointing towards the exhaust pressure sensor and when I checked it I found this

Anyone knows why this happened? And what the part number is? Doesn't look like exhaust is leaking out so hopefully I can drive it until the new sensor comes in..

Was that little chrome heat wrap sleeve fully slid over the sensor. That thing just to the left of your pointer finger in the pic?

the foil cover was all the way on. I'm thinking I'm supposed to use that tube to extend the sensor away from the manifold where its currently mounted since I have the Mpd turbo kit. Seems like a plastic sensor mounted right on the exhaust manifold wouldn't last long at all. Anyone have any part numbers? Having trouble finding them
