Oil in coolant?

A friend of mines 6.4 is blowing a engine oil/coolant mixture out of the overflow cap. I was originally thinking oil cooler but I'm not sure it would pressurize the coolant system like that. It doesn't have excessive crank case pressure. The truck has been dpf and egr deleted for awhile. The truck has just shy of 100000 miles. He was pulling a camper when it happened said the hottest oil temp he saw was 220 with coolant around 210. I think he was running spartan 210. He's currently down in far south west Texas and we're trying to get some ideas of what's wrong with it to determine what he does with the truck. Could it be head gaskets? Oil cooler? Thanks!

More than likely it will be the oil cooler is bad. They are pretty cheap to get. Cheaper from sunrise ford, they are a sponsor here

Yeah I'm leaning towards oil cooler. This truck has always seemed to run a little warm on oil temp but I did a bunch of coolant flushes with vc9 about 15-20000 miles ago and it seemed to help. From what I heard the overflow bottle is just blackish looking inside.
